Fellowship Application Fees (nonrefundable)


This fee is submitted upon completion of the application. (Exception: Associate Fellows do not have to submit the application fee.)

 

U.S. and Canada $200

 

International application fees are based on the World Bank income categories for each country:

 

  • High and upper middle income countries $100

  • Low and middle income countries $50

  • Low income countries $25

 

Initiate Fee: $375 (U.S. and Canada)

This fee is due upon notification of approval for Fellowship and granting of Initiate status.

 

2016 Dues (to be paid in U.S. dollars)

U.S. Fellows and Fellows of U.S. Territories and Possessions: $587

 

Canadian Fellows: $419

 

Dues for Fellows from countries other than the U.S. and Canada are based on the World Bank income categories for each country:

 

  • High and upper middle income countries $200

  • Low and middle income countries $100

  • Low income countries $50

 

Dues are not charged for Fellows who:

 

  • Have paid dues through the year of their 70th birthday (50 percent reduction granted after age 65).

  • Have been granted “retired” status. Retired status is defined as an individual who is not in the active practice of providing surgical or nonsurgical patient care, participating in funded research, or performing compensated teaching or administrative duties.

  • Are bona fide missionaries.

  • Office/business manager, surgical technologist, medical association personnel, residency program coordinator: $100

Dues statements are mailed in April, and dues are payable by July 1.
